Waiver & Release Form

ASSUMPTION OF RISK AND WAIVER OF LIABILITY By signing this waiver, I acknowledge that participation in activities at this facility involves inherent risks, including but not limited to: physical injury, sprains, strains, fractures, and other bodily harm. I understand that: 1. I am voluntarily participating in these activities with full knowledge of the dangers involved. 2. I assume full responsibility for any injuries or damages that may occur. 3. I release, waive, and discharge the facility, its owners, employees, and agents from any and all liability. 4. This waiver applies to me and any minors for whom I am signing. 5. I am physically fit to participate and have no medical conditions that would prevent safe participation. 6. I agree to follow all posted rules, guidelines, and staff instructions. 7. I consent to receive emergency medical treatment if necessary. I have read this agreement, fully understand its terms, and sign it freely and voluntarily. I understand this is a legally binding release of liability. This waiver remains in effect for 12 months from the date of signature
